Options for completing forms :

1. Complete forms electronically, print and sign (Only forms A, B and E need to be signed. Form B requires a signature twice.)  Either scan and e-mail to the Education Assistant OR send in hardcopy to temple.

2. Complete all forms electronically; digitally sign forms A, B and E and send via e-mail to the Education Assistant. If you want to learn how to digitally sign a form in Adobe Reader, you can watch this video.

3. Print forms and complete by hand. Either scan and e-mail to the Education Assistant OR send in hardcopy to temple.

Payment of the $125 deposit can be made by check or via Paypal (with a pass-through convenience fee of $5.00 – click button below).
